Second Visit - 4 weeks after New Patient Visit

This is your second comprehensive visit with the provider for 45+ minutes who will work with you to unlock a personalized journey to wellness. During your second visit, we evaluate the strides you've taken since your initial consultation. We will dive deep into your lab findings, and tailor a more comprehensive treatment plan that aligns with your unique needs.
But that's not all – you will also receive a 30 minutes-complementary consultation with our nutritionist who will explore the vital role nutrition plays in your well-being, gaining valuable insights and provide guidance to enhance your path to optimal health.

Lab Testing and Payment:
At Clark Wellness PLLC, we recommend that you pay for your functional medicine lab tests directly to us. Why? Because we have access to discounted rates for lab testing, and this can often result in cost savings for you.
Here's how it works:
- When you pay for labs through us, you benefit from our discounted rates, which are typically more affordable than what you might pay through conventional insurance-based healthcare.
Insurance Reimbursement:
You may submit the lab expenses to your insurance ( with the exception of Medicare) for reimbursement after paying us and getting your invoice. However, there is a caveat to keep in mind. Our lab testing approach is often more comprehensive and focuses on root causes, which may lead to a more extensive panel of tests compared to conventional models. As a result, there is a risk that your insurance provider may consider some of our tests as medically unnecessary. Also, our providers are considered out of Network. This could result in higher out-of-pocket expenses if your insurance decides not to cover them fully. If you choose to ask your PCP to order the panel, the likelihood of them leaving something out is huge, because many times they are not used to ordering such tests regularly and it increases the margin of error and may cause more frustration and delay in your treatment.
Can I use my insurance for my labs? Yes, but If you choose to do labs through your insurance, you will be opting not to use our discounted rate with our affiliated labs such as Access lab, Quest or Hamilton Lab.
In order to have your labs order sent to the lab of your choice, there is a processing and interpretation fee charge of $100. This fee covers the administrative work and interpretation of the results.

Functional Medicine Testing is expensive, can I make payments towards them?
-Yes, If you are having a hard time paying for your labs, we have a company called Rupa Health that allows you to make payments towards your labs.
Important Note Regarding MEDICARE:
It's essential to understand that our providers have opted out of Medicare, therefore, you must understand that you MAY NOT file with Medicare for any or the services provided by our practice for reimbursement.

**Every time your provider makes an adjustment in your hormones or your thyroid prescription, you’ll be requested to have subsequent labs.

Medical question - Outside lab review - Interim Visits
$100
If in-between visits you have a medical question for the provider or a medical concern to discuss with the provider instead of the medical team, or an outside lab to be reviewed, you will be charged $100 if the encounter does not take more than 15 minutes. If the encounter takes more than 15 minutes, it will be considered a visit and the charge will be $195.

Good Faith Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services.
You can ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.

Weight control is impacted by many factors beyond calories and physical activity. Gut Health
We will evaluate through stool analysis, breath test, blood work etc. We treat SIBO, food sensitivities, leaky gut, chronic diarrhea, yeast overgrowth etc.
We use GI Effects Comprehensive Stool Profile is one of the tests we can order and it can reveal important information about the root cause of many common gastrointestinal symptoms such as gas, bloating, indigestion, abdominal pain, diarrhea, and constipation. This stool analysis utilizes biomarkers such as fecal calprotectin to differentiate between Inflammatory Bowel Disease (IBD) and Irritable Bowel Syndrome (IBS). In addition, Genova's GI Effects test can be used to evaluate patients with a clinical history that suggests a gastrointestinal infection or dysbiosis.
Gut microbes are codependent with one another and with their human host, and the health of one affects the other. A sizeable volume of research associates a dysbiotic, or imbalanced gut microbiome with multiple disease states both within and outside of the GI tract. The diverse metabolic activities of the microbiome ultimately impact the human host, and the activities of the human host ultimately affect the health of their microbiome.

Adrenal Dysfunction is often a product of chronic stress exposure or history of stressful childhood. At your appointment, we will dive into your personal history, look at your current nutrition, activity behaviors, your current environment and exposure to toxic elements, psychological and emotional factors, your genetics etc.
The Provider will then order the appropriate diagnostic testing that examines deficiencies, intolerances, imbalances, and infections alongside your other labs to give us the best insight.
STEP 4
Once your deposit is received and your appointment is scheduled, you will receive an invitation to our patient portal where you will be able to upload your demographics, your picture and your medical records. You will also need to complete the requested questionaries.
